School transportation forms for the 2025-26 school year are due on July 1, 2025.
NO FORM NEEDED: If a student in grades K-9 will ride the bus to and from the stop nearest home, a form does NOT need to be completed.
FORM NEEDED: If a student in grades K-9 will never ride, or will only ride one way, attend MAC, or use a daycare/alternate address, a District Transportation Request Form must be completed. A District Transportation Request Form must be completed for resident students in grades 10-12 who intend to ride the bus.
FORM NEEDED: All non-Mahtomedi district residents (open-enrolled families) who are interested in transportation for their child(ren) must complete the District Transportation Request Form.
